Originating in the 1960s, Medicare is a government-provided fee-for-service health insurance program designed primarily for individuals aged 65 and older, as well as certain younger individuals with qualifying disabilities. The program consists of several distinct parts, each offering different types of healthcare coverage.

  • Medicare Part A: covers inpatient hospital care, skilled nursing facility care, hospice services, and certain home healthcare services.
  • Medicare Part B: covers outpatient medical services, including doctor visits, preventive care, vaccinations, laboratory tests, mental health services, outpatient procedures, durable medical equipment, and many other medically necessary services not provided during a hospital stay.
  • Medicare Part C: (Medicare Advantage) combines the benefits of Medicare Parts A and B and often includes additional benefits, such as prescription drug coverage.
  • Medicare Part D: provides prescription drug coverage through private insurance plans approved by Medicare.
  • Medicare Supplement Insurance (Medigap): is offered by private insurance companies and helps pay certain out-of-pocket costs not covered by Original Medicare, such as deductibles, copayments, and coinsurance.

Late Enrollment Penalties.

If you do not enroll in Medicare Part B during your Initial Enrollment Period, you may be subject to a late enrollment penalty. Your Initial Enrollment Period begins three months before the month you turn 65, includes your birth month, and ends three months afterward. Enrolling early can help you avoid unnecessary penalties.

  • You’ll pay an additional 10% of the standard Part B premium for every full 12-month period you were eligible but did not enroll.
  • The penalty is added to your monthly Part B premium.
  • It is not a one-time fee.
  • In most cases, the penalty lasts for as long as you have Medicare Part B.

Example:

  • In 2025, the standard Medicare Part B premium is $185.00. If you delay enrollment by two years, your penalty would be 20%, or $37.00 per month. Your total monthly premium would be $222.00, and this penalty generally continues for as long as you have Part B.

Generally, you won’t have to pay the Part B late enrollment penalty if you qualify for a Special Enrollment Period.

Example:

  • If you continue working beyond age 65 and remain covered under your employer’s or union’s health plan, you may qualify for a Special Enrollment Period when you retire. Before leaving your employer’s coverage, you should notify Social Security of your anticipated retirement date and begin the Medicare enrollment process approximately 3–4 months in advance to help ensure uninterrupted coverage.

A similar late enrollment penalty applies to Medicare Part D prescription drug coverage. The penalty is generally 1% of the national base beneficiary premium for each month you were eligible but did not enroll, and it is typically added to your premium for as long as you have Part D coverage.
